Description of Changes Requested: This request updates questions mentioning DEI or DEIA used in the Monthly Indicator Data Collection and Interview Protocols for CDC-RFA-DP-23-0002 Healthy Schools Program Evaluation​ (OMB control # 0920-1455) to be in accordance with EO 13985.
