Request: The Health Resources and Services Administration (HRSA), Maternal and Child Health Bureau requests approval for changes to the Healthy Start Evaluation and Quality Improvement information collection (OMB No. 0915-0338, expiration date September 30, 2026).


Purpose: HRSA is requesting a change request to revise questions to align with Executive Order 14168 Defending Women From Gender Ideology Extremism and Restoring Biological Truth to the Federal Government. This memorandum explains the changes and supporting rationale.


Changes: This request updates questions used in Healthy Start Demographic and Parent/Child Forms and Implementation Guides to be in accordance with Executive Order 14168. This request involves the following:

  • Revision of an existing questions

  • Deletion of an existing questions


Changes are outlined in detail in the table below, and in track changes versions of the forms (see Attachments).


Time Sensitivity: HRSA requests approval within the standard 10-day period to avoid any delay to data collection under 0915-0338.


Burden: The proposed changes are not expected to have any impact on burden.
