Affidavit of Ownership for Unmanned Aircraft (UA)
PRA Public Burden Statement: A federal agency may not conduct or sponsor, and a person is not required to respond to, nor shall a
person be subject to a penalty for failure to comply with a collection of information subject to the requirements of the Paperwork
Reduction Act unless that collection of information displays a currently valid OMB Control Number. The OMB Control Number for
this information collection is 2120-0042. Public reporting for this collection of information is estimated to be approximately 30
minutes per response, including the time for reviewing instructions, searching existing data sources, gathering, and maintaining the
data needed, completing and reviewing the collection of information. All responses to this collection of information are required to
show applicant eligibility and evidence of ownership to register an aircraft or hold an aircraft in trust in accordance with 14 CFR Part
47. Send comments regarding this burden estimate or any other aspect of this collection of information, including suggestions for
reducing this burden to: Information Collection Clearance Officer, Federal Aviation Administration, 10101 Hillwood Parkway, Fort
Worth, TX 76177-1524. This form may be filled out and submitted online at: CARES.FAA.gov.
Information: The Secretary of the Department of Transportation (DOT) and the Administrator of the Federal Aviation Administration
(FAA) affirmed that all unmanned aircraft, including recreational small, unmanned aircraft, are aircraft. As such, in accordance with
49 U.S.C. 44101(a) and as further prescribed in 14 CFR part 47, registration is required prior to operation. See 80 FR 63912, 63913
(October 22, 2015).
Preparation: All items must be completed, and the affidavit must be signed and notarized. When choosing a serial number, only
show one depending on the UA and whether it is remote ID equipped. For UA with Remote ID, it must be the serial number of the
standard remote identification UA or, if equipped, the serial number of the remote ID broadcast module. For UA without Remote ID,
it must be the manufacturer assigned serial number (See 14 CFR §47.14).
Submission Instructions: After entering all required data and affixing the necessary signature and notary, send it (IF INK
SIGNATURES ARE AFFIXED, THE ORIGINAL INK-SIGNED COPY MUST BE SENT) to: if by USPS/Mail, FAA Aircraft
