This letter certifies that the US Department of Agriculture’s Food and Nutrition Service has contracted with Westat Incorporated and Estudios Técnicos, Inc (ETI) to conduct the Puerto Rico Health and Well Being Study (Contract Number: GS-10F-0136X). This contract requires data collectors to deliver a survey questionnaire to selected households and answer questions about the study. The data collectors may also return to pick up completed surveys, at the households’ request.
I am writing to request your cooperation for this important study by allowing data collectors to deliver paper questionnaires to selected households. All data collectors wear a photo ID badge that identifies them as a study representative.

	information is being collected to assist the Food and Nutrition
	Service (FNS) in understanding food security status and economic
	well-being among Puerto Rico residents. This is a voluntary
	collection. FNS will use the information as a baseline for future
	assessments of food security and the Nutrition Assistance Program,
	particularly in the context of natural disasters. This collection
	does not request personally identifiable information under the
	Privacy Act of 1974. According to the Paperwork Reduction Act of
	1995, an agency may not conduct or sponsor, and a person is not
	required to respond to, a collection of information unless it
	displays a valid OMB control number. The valid OMB control number
	for this information collection is 0584-0674.
	The time required to complete this information collection is
	estimated to average 5 minutes per response, including the time for
	reviewing instructions, searching existing data sources, gathering
	and maintaining the data needed, and completing and reviewing the
