Justification for the Change – The request for a nonmaterial/non-substantive is explained in table format shown below:

Justification for Change

This is a request for nonmaterial/non-substantive changes as shown above to APHIS/CDC Form 2 -Request to Transfer Select Agents and Toxins, APHIS/CDC Form 4A - Reporting the Identification of a Select Agent or Toxin From a Clinical/Diagnostic Specimen, APHIS/CDC Form 4B -Reporting the Identification of a Select Agent or Toxin: Proficiency Testing Report, and APHIS/CDC Form 4C -Reporting the Identification of a Select Agent or Toxin: Federal Law Enforcement Seizure Report for OMB Control No. 0920-0576 Expiration 11/30/2015 used by the Centers for Disease Control and Prevention (CDC), Division of Select Agents and Toxins (DSAT) Form 2 and Form 4 Teams. No other changes have been made to the currently approved forms. Since the nonmaterial/non-substantive changes as explained above only consist of changing the fax number, there is no change to the burden or respondents completing the form.
