Crosswalk for Changes to CHIP SPA Template

Introduction	Updated the submission requirements to reduce the number of required hard copy submissions from 20 to one.	CMS does not need 20 hard copies.

Whole document	Combined the instruction document with the CHIP SPA template to have a single document.	States will have an easier time completing the template as the guidance will be more accessible.
Sections 1, 2, 4, 5, 6, 8, 9, 10, 12 (all these changes are highlighted in the document)	Incorporated the previously issued guidance and templates.	Includes all OMB-approved guidance in one place to ease the burden on the States so they don’t have to look in multiple places for approved templates to incorporate into their State plans.
Sections 2 and 5	Moved sections 2.2 and 2.3 into section 5	To eliminate redundancies between sections 2 and 5.
Section 4	Removed crowd-out language that was previously listed as sections 4.4.4.1.-4.4.4.3	The language was added as a result of a State Health Official letter and it was later repealed.
