Justification for 1405-0011 change request

The DS-2029 is being simplified by the elimination of the Social Security Application section of this form. The inclusion of the SS application was done as a courtesy to respondents so they could complete both a birth abroad report and a slightly reformatted social security number application at the same time. The Social Security Administration has changed its procedures for application for a Social Security Card for minors.  SSA will no longer accept page 2 of the DS-2029 as an application for a Social Security Card for a minor born abroad.

This will not affect the burden associated with this collection, as the burden of the SS application was not included because the respondent did not have to fill out the SS section to report a birth abroad. The SS section was returned to the SSA, not the Department of State.
