This survey is in conjunction with a producer survey regarding sales of flowers and nursery products. Your information is kept strictly confidential and cannot be obtained by anyone, including any other government agency. This confidentiality of information is protected by Title 7 of the United States Legal Code under Chapter 55, Section 2276. Individual reports are used only in combination with reports from other producers to prepare state and national agricultural estimates. Response to this survey is voluntary and not required by law. However, cooperation is important in order to estimate accurately flower and nursery exports in Hawaii. Please return one (1) copy of this questionnaire in the enclosed self-addressed envelope which requires no stamp. Provide your best estimate if book figures are not available.
