OMB # 0960-0105
Revision to the Collection Instrument
Explanation of the change:
In the attached revision, we eliminated the highlighted language between the sections “For you to receive student benefits after age 18, you must:” and “To receive childhood disability benefits, you must contact any Social Security office and have the following information”:
NOTE: If you mail the form, a Social Security representative will contact you to discuss your rights and responsibilities as a student. This discussion must take place before we can process your student award.
Rationale for the change:
We will no longer contact the student to discuss his/her rights and responsibilities as a student, and the form should reflect SSA’s current policy and procedure.
The change to eliminate the personal contact from the Title II student process, and therefore the need to eliminate language from Form SSA-1372, is a recent initiative intended to streamline processing applications for student benefits. At the time we submitted the clearance package on February 6, 2008, this change was not anticipated which is why we are submitting it now.
