Appendix A: Modifications to the UDS

Table 3: Part B: Users by Race/Ethnicity and Language: Added category “More than one Race” to permit multiple race reporting in aggregate totals.

Table 4: Socioeconomic Characteristics: Added category “Unknown” Insurance: this category will allow grantees to accurately report patients for whom principal source of insurance for primary medical care services is not known.

Table 5: Added category: "Total Midlevel Practitioners": This line automatically sums lines 9 and 10. The addition of this line does not require the grantee to calculate, as this will be automatically calculated upon data entry. There is no additional reporting burden for grantees.

Added category: "Eligibility Assistance Workers" on line 27a, to match the FTE reporting to what is reported in Table 8A, detailed cost information for FTEs. Table 5 and Table 8A are now consistent in FTE reporting.

Renamed line 25 from "Education Specialist" to “Patient and Community Education Specialists”, to match the categories of health education staff in Table 8B. Table 5 and Table 8B now have consistent categories of health education staff.

Table 6: Selected Diagnoses and Services Rendered: Updated the CPT, ICD9 and ADA Codes using the current year billing code manuals. Revised codes are corrected on Lines 1, 6, 9, 22, 24, 28, 29 and 31.
